Abuse & Neglect

Recognizing the need for increased coordination among agencies and/or additional services to address abuse and neglect in Kosciusko County, the Kosciusko County Community Foundation and K21 Health Foundation have established a special grant initiative to seek out proposals from local service providers. This initiative aims to establish new or support currently existing services, programs, or projects that meet the needs of Kosciusko County individuals, children, or families who have already experienced or are at risk of experiencing abuse and neglect. Any proposal submitted must describe how it serves this specific population.

Applications will be accepted until March 1, 2025, with awards being announced in mid-May.  Please review the Request for Proposals document below for the full guidelines.

HOW TO APPLY

Grant seekers should review the Request for Proposals, which includes the priorities and guidelines for this initiative, posted below before applying.  You are also welcome to preview the applications by downloading the preview application.  Please note: Official applications must be submitted on the grant portal.

Grant seekers are required to contact Alex Hall (alex@kcfoundation.org), vice president of programs, to set up a time to discuss your proposal prior to submitting a formal application online.

  • IRS Determination Letter (required for 501c3 organizations)
  • List of board members with city/town of residence
  • List of staff members with city/town of residence
  • Financial documents*

*What financial documents should I include with my application?

You will need to upload your organization’s most recent Statement of Activities and Statement of Financial Position. Examples of both documents are below. Your organization’s most recent 990 is also requested, but not required.

FORMS FOR GRANT RECIPIENTS

Payment request(s) should be submitted through the online grant portal after the grant has been approved and following the guidelines on the grant portal and grant agreement.  You may be required to fill out a Request for Payment Form; however, this form will be linked in the grant portal as well as here on this webpage below:
